Here is a list of some of the items that do not have the x4 to their AC. If you could sticky this post Osiris I can add to it as people report any known items.

It's really arbitrary too. I can tell you the Bone Golem legs from Ssra have had their AC adjusted. Not sure what the methodology was on the original script that adjusted these items, but maybe it needs to be rerun from scratch.
I was never clear on what the deal was with "128" and stats. I presume that if the script hit a stat on an item that would kick an item over 128 with a stat, it didn't adjust the item.
With x4 instead of x5 maybe this wouldn't be a problem? At least with items through PoP?

I went back to Ssra Temple to test something (and kill Archlich).
The bone golem vambraces now have an AC of 464 if I read that correctly. Normally they are 29 ac, so should be 116 here.
Bone Golem greaves were showing up at 37, which is their normal value. I could have sworn these had already had the AC adjusted though.

Aged Robe of the Azure Sky from Phara Dar in VP also needs its AC adjusted.

AC has been adjusted on the following (a server reboot is required for changes to take effect)
bone golem vambraces AC = 116
Leggings of the Fiery Star AC = 240
Aged Robe of the Azure Sky AC = 160

I also updated the AC on quite a few other items as well. Reboot is needed for changes to take effect.

I noticed many of the BP's Trakanon drops need the AC adjusted. So far for sure the Cobalt BP and Blood Ember BP do not have the adjusted AC.
